how do mob bosses bring associates to turn insane?

I have hear that a person who looks into the eyes of a mob boss is condemed to a mental hospital. Can a creature be drugged or made to look insane without man insane I think they falsification their mental illness to avoid testify against the mob bosses and to avoid hard time contained by prison. I saw a special one one that was pretending to be menatally sick and when investigators asked him who the Presient was, during approaching the first Bush or Clinton years, he didn't answer the question. They continued speaking another ten to 20 minutes and the con artist blurted out "Eisenhower" or "Nixon" or some such balony. But the investigators be stunned he could remember the question that long if he be as out of it as he appeared to be.
