Currently, there are twelve states next to effective medical marijuana law on the books: Alaska, California, Colorado, Hawaii, Maine, Maryland, Montana, Nevada, Oregon, Rhode Island, Vermont, and Washington. It should be noted that Maryland's statute does not legalize possession of medical marijuana, but a bit makes it a non-incarcerable offense near a maximum penalty of a $100 fine.
In Hawaii, "patients diagnosed next to the following illnesses are afforded legal protection lower than this act: cachexia; cancer; chronic spasm; Crohn’s disease; epilepsy and other disorders characterized by seizures; glaucoma; HIV or AIDS; multiple sclerosis and other disorders characterized by muscle spasticity; and nausea. Other conditions are subject to approval by the Hawaii Department of Health. "
Application information for the Hawaii medical marijuana registry is available by writing or calling:
Hawaii Department of Public Safety
919 Ala Moana Boulevard
Honolulu, HI 96814

I own attached sources for your reference.
